This is an interstage transformer for the Saku-ma amplifier, specifically for the 5691 tube.
It was purchased about 30 years ago for use in an all-transformer-coupled mono power amplifier.
This is a custom-made transformer from Sun Audio, and it's likely a rare item that's no longer available.
I'm attaching the Tamura quality assurance record.
Lot 03377, manufactured in 1991.
The box is old, but it's the original one from when I purchased it.
Transformer specifications:
100KΩ : 200KΩ, both primary and secondary are split.
Size: Approximately 72mm in height and 50mm in diameter on the chassis.
